Clinical Studies on the Ascitie Gamma-glutamyl Transpeptidase Levels in Primary Hepatoma


Abstract
Gamma-glutamyl transpeptidase (r-GTP) is a enzyme, concentrated mainly in kidney, pancreas, liver, spleen, small intestine, brain and heart muscle as a very sensitive indicator of liver cell dysfunction, especially in primary hepatoma.
For the purpose of differential diagnosis of ascites with primary hepatoma, the levels of ascitic 7-GTP in various gastrointestinal diseases have been estimated, the correlation between ascitic 7-GTP and serum alkaline phosphatase, SCOT, SGPT activity, ascitic total protein -amount established. Also by peritoneoscopic examination the relationship between 7-GTP levels and extent of involvement of primary hepatoma has been analyzed.
The material included 19 cases of primary hepatoma, 18 cases of liver cirrhosis, 12 cases of tuberculous peritonitis, 18 cases of stomach cancer and 6 cases of benign intraabdominal tumor. The results are as follows.
1. The peak age incidence of primary hepatoma was in 6th decade and ratio between male and female was 2.8 : 1.
2. Mean values of ascitic r-GTP levels was highest in primary hepatoma(60.230.4mU/ml) and they were 28.3 14.2mU/ml in stomach cancer, 14.8 11.6mU/ml in liver cirrhosis, 14. 0 6. 6mU/ml in benign intraabdominal tumor and 10.2-F 5.2mU/ml in tuberculous peritonitis in decreasing order (P<0.05).
3. The correlation between ascitic 7-GTP and serum alkaline phosphatase, SGOT, SGPT
activity, ascitic total protein amount were absent in primary hepatoma (P>0.05).
4. Mean values of ascitic 7-GTP levels in primary hepatoma with and without liver cirrhosis
were 61.228.2mU/ml and 59.832. 1mU/ml, respectively. The difference of both cases
t! was not statistically significant (P>0.1).
5. Primary hepatoma was divided into three groups by extent of involvement under peritoneoscopy. Mean values of 7-GTP levels in group A was 98.638.7mU/ml, in group B 49.8-!-4.OmU/ml and in group C 38.03.6mU/mI, respectively (P<0.05).
